Become a Mentor
By becoming Mentor, you’ll have the opportunity to make a difference in an individual’s life. You’ll also receive the tools, education and support from our expert team that you need to be successful.
Adult Foster Care
In addition to providing shared living through our Host Home program, we also offer Adult Foster Care (AFC). We provide adults with disabilties and elders with complex medical conditions who receive Mass Health Insurance with caring, supportive homes. They live in the private family homes of their Mentors. With constant support from our team, Mentors help the adults we serve participate in home and community life.
Personalized Services
As with all our residential programs, adults in our AFC program are actively involved in planning their daily routine and the services they need to be successful. Our AFC program includes:
- 24-hour support in family homes
- Life skills development
- Opportunities to participate in community activities and day programs
- Advocacy services
- Community integration
- Family involvement and support
- Medication management oversight
- 24-hour on-call support for Mentors
- Quality improvement
We also provide Family First Adult Foster Care. In this program, the individual’s relative – who may already be their caregiver – becomes their certified Mentor.
