Transition Services
We help young people with intellectual and developmental disabilities transition to the adult services that are right for them.
Our team works with young adults (ages 18 – 22), their families, care providers, and our public partners.
Together, we map out individual service plans. The plans outline personalized supports to help individuals reach their personal goals. Whether they dream of going to school, getting a job, or finding the home of their own, we help set them on the path to the future of their choice.
While the individual remains in their current living situation, we coordinate supports for the next chapter of their lives. We then transition them into the adult services that will help them continue to grow, learn, and thrive.
